Important Note 重要提示：

1. Your request is subject to approval by 

the Company. If the request is approved, 

it will be recorded and duplicate copy of

this form will form part of the policy. 

阁下之申请需经本公司审批，如阁下之申请获批准，有关申请将被记录，而此表格之副本将成为本保单的一部分。

2. Any changes must be signed by the new policyholder. 

任何答桉如有更改，敬请新保单持有人在旁签署。

3. Please complete this form along with the 

“Tax Residency Self-Certification Form(s)” 

signed by the new policyholder. 

请填妥此表格连同由新保单持有人签署的《税务居民自我证明表格》一併提交。

4. Both new and existing policyholders are required to 

undergo our due diligence process, and any request for the transfer of

policy ownership shall be subject to the Company’s 

sole discretion and approval. 

现有及新保单持有人均需通过我们的审查程序，任何关于保单权益转让的请求均需经公司全权酌情决定及批准。

5. Some policy benefits linked to the existing policyholder

 (like Death Benefit Settlement Options and others) will end when

the ownership is transferred. The new policyholder will 

need to re-apply for these benefit(s) in his/her name as appropriate.

个别与现时保单持有人相关的福利（例如身故赔偿支付选项等）将在保单权益转让后结束。新保单持有人需根据情况重新以其名义申请福利。
