STAFF NOTIFICATION 员工须知

With a view to guaranteeing staff’s health and life and 
alleviating their economic losses when an accident or 
illness occurs, the company has purchased GC Group Worldwide 
Medical Plan (including out-patient,in-patient and dental 
benefit) from XX Life Insurance Company Shanghai Branch 
(hereinafter referred to as “XX”); 

公司为更好地保障员工在患病或发生意外时的经济利益，为您在XX人寿保险有限公司（以下简称“XX人寿”）投保了XX全球保障团体医疗保险（包括门诊，住院及牙科医疗责任）。
All contents in this scheme are based on the insurance contract 
between “XX” and “XX”, the insurance period is from Sep. 
01, 20XX to Jun. 30, 20XX.

所有保障内容以“XX人寿”与“XX”签订的保险合同为准，保险期间为20XX年09月01日到20XX年06月30日。
Please read this scheme carefully. Should you have any questions 
regarding to the contents, please refer to customer service from XX.

Claim Query理赔查询
1.    E-mail Notice 电子邮件通知
When claim processing is completed, XX will send an electronic 
edition of “Claim Decision Notice” to staff by email.
在理赔完成后，XX人寿将通过电子邮件形式，向员工发送电子版本的《理赔决定通知书》。
2.    SMS Notice手机短信通知：
When claim processing is completed, XX will automatically 
send SMS to staff.
在理赔完成后，如果员工选择使用短信通知，XX人寿将通过手机短信通知员工。
3.    Hardcopy Claim Notice纸质版理赔通知书：
